📝 Terms of Service for TRIVISIT Packages

Welcome to KLINIK TRICLINIC. When you purchase any TRIVISIT Package (BASIC, PLUS, or MAX), you agree to the following terms of use.

📦 What You’re Getting

Each TRIVISIT Package includes 3 full clinic visits, valid for:

• Consultation with a certified doctor

• Essential diagnostics (based on your package level)

• Prescribed medications

• Follow-up teleconsultation access

Each visit deducts from your 3-visit balance. No top-ups or hidden fees unless stated.

🕒 Validity Period

• All packages are valid for 12 months from the date of purchase.

• Any unused visits after 12 months will expire. No refunds for expired visits.

🔁 Transferability

• Packages are fully transferable to immediate family or friends.

• Just inform our team ahead of the visit to assign a name to the visit record.

🧪 TRICLINIC Blood Test Tier System

All in-house tests are grouped into three color-coded tiers based on complexity and value. This helps patients see what’s included — and reassures you that no extra payment is needed if you’re on a TRIVISIT plan.

⬜ WHITE TIER – Everyday Essential Tests

• Single Test Price: RM50

• TRIVISIT Package Price: RM25 (deducted from your balance)

✔️ Influenza A/B (Swab)
✔️ Dengue NS1 / IgG / IgM
✔️ TSH, Free T4 (Thyroid)
✔️ Kidney, Liver, Pancreas Function

✔️ HbA1C
✔️ Cholesterol Profile
✔️ HBsAg / Anti-HBs / Anti-TP

🟨 YELLOW TIER – Hormones & Tumor Markers

• Single Test Price: RM70

• TRIVISIT Package Price: RM35 (deducted from your balance)

Common tests:
✔️ FSH, LH, Estradiol, Progesterone
✔️ Anti-HCV / Anti-HIV
✔️ Ferritin
✔️ Total PSA
✔️ Total IgE (Allergy marker)
✔️ CA19-9, CEA, AFP

🟥 RED TIER – Advanced / High-Value Tests

• Single Test Price: RM100

• TRIVISIT Package Price: RM50 (deducted from your balance)

Common tests:
✔️ Anti-Mullerian Hormone (AMH)
✔️ Beta-HCG
✔️ Testosterone
✔️ D-dimer
✔️ Cardiac Troponin I
✔️ CA125, CA15-3, CA19-9
✔️ Imaging (X-ray, Ultrasound)*

*Only available under TRIVISIT MAX / Plus (with discount)

✅ TRIVISIT Package Reminder:

• You only pay once — at the start.

• No extra charges for tests inside your plan.

• You’ll always be informed of how much is being deducted, not added.

🖥️ Telemedicine Access

• Available for all TRIVISIT packages.

• Can be used for follow-ups, test reviews, sick leave extensions, medication queries, or arranging free medication delivery.

💊 Compounded Medication Access

• TRIVISIT BASIC: Covers standard medication only

• TRIVISIT PLUS & MAX: Includes access to custom medication strips like TRIBOOST, TRICLEAR, TRISOOTHE

• These medications are doctor-prescribed and safe, compounded in licensed Malaysian compounding pharmacy labs.

🧾 Refund & Cancellation

• All TRIVISIT sales are final.

• Packages are non-refundable once activated (i.e., once the first visit is used).

• If you’ve purchased a package in error, contact us before your first visit for assistance.

⚠️ Fair Use Policy

TRIVISIT is meant to make care affordable and accessible — not to overload our system. We reserve the right to limit abuse, including:

• Frequent same-day usage

• Repeated transfers for non-family use

• Use for non-medical purposes (e.g., non-diagnosed supplements)
